Description: About High Plains Community Health Center: High Plains Community Health Center is a Federally Qualified Health Center (FQHC) located in Lamar, CO. Patients are served from all over Southeastern Colorado including Prowers, Baca, Bent and Kiowa Counties. In calendar year 2008, High Plains provided primary care services to 8,408 unduplicated patients during 30,933 visits. This included 23,110 visits to 6,969 medical patients and 7,239 visits to 2,931 dental patients. We also have a full-time mental health clinician stationed at our main medical facility for the purpose of integrating physical health and behavioral health, an in-house pharmacy, and a very dynamic preventive and chronic care team including a Health Educator. Comprehensive primary care services provided on-site includes acute medical and dental care, well-child exams, pharmacy, health education, case management, immunizations, x-rays, laboratory, preventive and restorative dental care, chronic illness care, well woman exams, and individual, group and family psychotherapy. We are staffed with three physicians, three nurse practitioners, two dentists and two dental hygienists. We provide basic entry level care to all ages in all three arenas of medical, oral and mental. All services are on an outpatient basis with charges based on an ability to pay.

Description: Responsibilities: The Health Coach will assist chronic disease patients in reaching their self-management goals using a variety of health behavior techniques including Motivational Interviewing and counseling based on the Transtheoretical Model of Health Behavior Change. Self-management goals may include but are not limited to the following: exercise, weight management, healthy eating, disease management, smoking cessation, and stress management. The health coach will assist the patient in devising an individually appropriate action plan to achieve their goal. Support will be provided to the patient to help identify barriers to achieving their goals and strategies to overcome barriers. The health coach will connect the patient with clinic and community resources needed to achieve their goal. The health coach will be an integral part of a multidisciplinary healthcare team, and effective communication with the patient, the clinical provider and other team members is imperative. The health coach must be passionate about educating and helping others make positive behavior changes. Training and professional development opportunities will be available; some travel may be required.
   
Compensation: $40,000/year + benefits
Qualifications: •Bachelor’s degree (health-related field preferred) relevant experience in behavioral change intervention •Bilingual in Spanish preferred •Sensitivity to low-income and cultural issues •Empathetic, patient-centered approach •Excellent communication, listening, and assessment skills •Self-directed and highly organized •Ability to work effectively in teams •Computer literate
