Description: Excellent job opportunity for a highly motivated individual to provide smoking cessation counseling and asthma/COPD self-management education for adults and children in clinical and community settings. This individual will be hired by Respiratory Health Association of Metropolitan Chicago (RHAMC) to work for the Cook County Hospitals and Health System (CCHHS) Healthy Lungs Initiative, which provides asthma and COPD self-management education and smoking cessation services to residents of Cook County. Responsibilities may include: - Providing smoking cessation counseling and resource referrals to patients in hospitals, clinics, and community agencies according to program protocols; - Providing asthma and COPD education, including guidance on medications, devices, and performing spirometry assessments, to patients in hospitals, clinics, and community agencies according to program protocols; - Traveling to clinical and community settings throughout Cook County for service delivery; - Conducting telephone-based smoking cessation counseling and leading group smoking cessation sessions; - Identifying and recruiting patients, and conducting follow-up at designated intervals; - Communicating with physicians and other clinical staff on patient status and progress; - Working directly with Clinical Supervisor, CCBHS and RHAMC staff, and other clinical staff and program collaborators to facilitate service delivery; - Documenting and tracking patient number and nature of patient encounters; - Collecting and organizing program data; inputting data in web-based program database; - Attending initial asthma, COPD, and smoking cessation trainings, and ongoing trainings as needed; and - Performing other duties as necessary.

Qualifications: Minimum Requirements: - Bachelor’s degree required. - Bilingual – English/Spanish or English/Polish required. - Previous work experience providing health or behavior change education/counseling required. - Previous work experience demonstrating excellent interpersonal communication skills required. - Previous work experience demonstrating ability to self-start and to organize information required. - Previous work experience demonstrating ability to prioritize and perform multiple tasks required. - Ability and willingness to travel to clinical sites around Cook County, by car or public transit, required. - Knowledge of asthma and/or tobacco control preferred. - Experience working with underserved communities preferred. - Experience working within a multidisciplinary health care team desirable. - Experience and/or knowledge of motivational interviewing or harm-reduction counseling methods desirable - Intermediate computer skills and proficiency using Microsoft Office, especially Excel, preferred. - Flexibility in hours and locations of work is highly desirable.
